We must follow the privacy practices that are described in this Notice while it is in effect.
This Notice takes effect on the date listed above and will remain in effect until we replace it. We reserve the right to change our privacy practices and the terms of this Notice at any time, provided such changes are permitted by applicable law, and to make new Notice provisions effective for all protected health information that we maintain. Some information, such as HIV-related information, genetic information, alcohol and substance use disorder treatment records, and mental health records may be entitled to special confidentiality protections under applicable state or federal law.
We may use and disclose your health information for your treatment. For example, we may share your health information with a referring dentist, a specialist involved in your care, or a laboratory performing diagnostic tests. We may also disclose information to other health care providers who are treating you or consulting on your case.
We may use and disclose your health information so that we can bill for the treatment and services you receive and collect payment from you, your insurance company, or a third-party payer. For example, we may need to provide information to your insurance company to receive pre-approval for a planned endodontic procedure or to verify your coverage and benefits.
We may use and disclose your health information for our health care operations purposes. These activities include quality assessment, staff review, training, licensing, and conducting or arranging other business activities. For example, we may use your health information to evaluate the performance of our clinical team in caring for you.
We may use or disclose your health information when we are required to do so by federal, state, or local law. For example, we may disclose health information when required by a court order in a legal proceeding, or to report certain types of wounds or diseases as required by public health authorities.
We may disclose your health information for public health activities, including reporting disease, injury, vital events such as births, and conducting public health surveillance and investigations.
